STRONGHER TOGETHER PARTICIPANT AGREEMENT, LIABILITY WAIVER & TERMS OF PARTICIPATION

Program Description:
StrongHer Together is a women’s wellness and support community designed to promote encouragement, healthy movement, low-impact exercise, stretching, dance fitness, sound relaxation, community building, and emotional support in a faith-friendly environment.

Participation is voluntary.

By checking the box below, I acknowledge that I have carefully read, understood, and agree to the following:

1. VOLUNTARY PARTICIPATION

I understand my participation in StrongHer Together activities—including but not limited to:

  • Walking exercises

  • Dance movement

  • Stretching

  • Low-impact fitness

  • Resistance activities

  • Breathwork

  • Relaxation exercises

  • Sound therapy/meditation

  • Group discussions

  • Wellness activities

  • Refreshments/social gatherings

is voluntary.

I choose to participate at my own discretion.

2. MEDICAL ACKNOWLEDGEMENT

I affirm that:

□ I am physically able to participate.

OR

□ I have consulted with my physician regarding participation.

I understand HOME Christian Church, StrongHer Together, volunteers, facilitators, instructors, and affiliates do NOT provide medical advice.

Participation should not replace:

  • Physician care

  • Mental health treatment

  • Physical therapy

  • Counseling

  • Medical treatment

I agree to stop any activity causing pain, dizziness, shortness of breath, or discomfort.

3. ASSUMPTION OF RISK

I understand exercise and physical movement carry inherent risks including:

  • Falls

  • Sprains

  • Muscle soreness

  • Injury

  • Elevated heart rate

  • Aggravation of existing conditions

I voluntarily assume all risks associated with participation.

4. RELEASE OF LIABILITY

To the fullest extent allowed by Missouri law, I release and hold harmless:

Pastors
Employees
Volunteers
Leaders
Facilitators
Property owners
Contractors
Representatives

from claims involving:

  • Personal injury

  • Illness

  • Property damage

  • Emotional distress

  • Accidents

  • Participation-related incidents

except where caused by gross negligence or intentional misconduct.

5. EMERGENCY MEDICAL CARE

In an emergency, I authorize staff or volunteers to seek emergency medical assistance on my behalf.

I understand:

I am financially responsible for any medical costs incurred.

6. WELLNESS SUPPORT IS NOT COUNSELING

StrongHer Together may include encouragement, faith discussion, support circles, or wellness conversations.

I understand these interactions:

ARE NOT:

  • Professional counseling

  • Therapy

  • Crisis intervention

  • Mental health treatment

If I experience emotional crisis, suicidal thoughts, abuse concerns, or severe mental distress, I agree to seek licensed professional help.

7. SOUND THERAPY ACKNOWLEDGEMENT

I understand relaxation music, sound bowls, meditation exercises, or calming experiences are intended for comfort and stress reduction only.

These activities:

  • Are optional

  • Are not medical treatment

  • Are not guaranteed to produce outcomes

8. FOOD & REFRESHMENTS DISCLAIMER

Refreshments may occasionally be provided.

I understand:

I am responsible for monitoring allergies, sensitivities, and dietary restrictions.

HOME Christian Church is not responsible for allergic reactions or dietary complications.

9. PHOTOGRAPHY & MEDIA RELEASE

StrongHer Together activities may occasionally be photographed or recorded for:

  • Church promotion

  • Social media

  • Marketing

  • Website content

  • Testimonials

Please choose:

□ YES — I allow my image/video to be used.

□ NO — I do not consent.

10. CONFIDENTIALITY & COMMUNITY RESPECT

To maintain a safe environment:

Participants agree:

✓ Treat others respectfully
✓ Avoid gossip
✓ Honor privacy
✓ Avoid sharing personal stories heard in group settings without permission

HOME Christian Church cannot guarantee confidentiality from other participants but encourages mutual respect.

11. CONDUCT EXPECTATIONS

Participants may be removed without refund for:

  • Harassment

  • Aggressive behavior

  • Disruptive conduct

  • Bullying

  • Intoxication

  • Unsafe actions

  • Repeated disregard for guidelines

12. MEMBERSHIP & PAYMENT TERMS

Membership:

$50/month

Membership may include:

  • Group sessions

  • Support activities

  • Wellness experiences

  • Refreshments

Payments:

□ Are non-refundable after the billing cycle begins

OR (adjust if preferred)

□ Refund requests reviewed case-by-case

Missed sessions do not guarantee makeups.

13. WEATHER/CANCELLATION POLICY

Sessions may be cancelled due to:

  • Severe weather

  • Facility issues

  • Emergencies

  • Instructor absence

Notice will be provided when possible.

14. NO GUARANTEE OF RESULTS

I understand outcomes vary.

HOME Christian Church does not guarantee:

  • Weight loss

  • Fitness improvements

  • Emotional outcomes

  • Healing outcomes

  • Specific wellness results
